Make an appointment for an Eye Exam

There are several ways to make an appointment at Zeker Zicht:

• With a digital referral via ZorgDomein from your GP. 
• With a paper referral letter (not via ZorgDomein). 
• Without a referral letter. 

Please see below for the cost differences between the different methods.

Digital referral via ZorgDomein

Do you have a digital referral via ZorgDomein from your GP?

If so, we will schedule your appointment shortly. You will be called by the appointment planner of Huisarts Plus Punt.

Paper referral letter (not via ZorgDomein)

Do you have a referral letter from your GP, but no digital referral via ZorgDomein?

If so, you can make an appointment in two ways: via our website or by phone.

Please note: Eye examinations with a paper referral letter are not fully reimbursed.

• Own risk: Approximately 70% of the costs will be charged to your own risk.
• Personal contribution: You will pay the remaining 30% yourself.

The total cost of an eye examination is €97.33. You will receive an invoice from Stichting Optonet that you can declare with your health insurance company. You will pay this invoice directly to Stichting Optonet.

Without a referral letter

Do you not have a referral letter?

If so, you can still make an appointment directly.

The cost of your eye examination depends on the type of appointment you have.
